The Center for Advanced Wound Healing and Hyperbaric Medicine provides on-site services at the Wyckoff Heights Medical Center. The center is located on the third floor of the “A” building. The center’s mission is consistent with the medical center’s mission to provide excellence in care through prevention, education and treatment in a safe environment.
The center provides services to adults and geriatric patients, primarily on an outpatient basis.
Scope and Complexity of Patient Needs
The scope of care includes evaluation and treatment of chronic wounds to a diverse and complex patient population of adult and geriatric patients. Care is designed to meet identified needs, both actual and potential. The complexity of the population served requires the coordination of various healthcare disciplines, utilizing appropriate wound management products and technologically advanced practices in a supportive environment. Patients enter the center via referrals from Ambulatory Services, private physician referral, as part of the discharge process of continuity of care and follow-up, community agencies and self-referral.
Methods Used to Assess and Meet Patient Care Needs
Services provided are designed to respond to patient needs and expectations. Mechanisms used to determine patient needs start at the time of referral. Staff is oriented to be sensitive to patient’s medical as well as socio-economic conditions by providing appropriate referrals and appointments without delay.
During the Patient/Family interview, which is conducted as part of the nursing assessment, and physician history and physical examination, data is collected and reviewed to determine a multidisciplinary plan of care.
Learning needs are also assessed in order to initiate appropriately and timely educational interventions.
Services are based on the comprehensive assessments, complete history and physical, wound assessment and description and diagnostic investigations.
Services are provided through an integrated team of skilled healthcare workers including: physicians, podiatrists, nurses and technicians. Dietary, Social Work Services, vascular labs, vascular surgery and Physical Therapy referrals are generated as appropriate. Wound care utilizes advanced, computerized, and standardized protocols with state-of-the-art equipment.
Recognized Standards of Practice
All personnel in the department are governed by the policy and procedure of Wyckoff Heights Medical Center as well as the recognized standards and guidelines from all regulatory agencies (NYS/NYC Departments of Health, Joint Commission, OSHA, CMS etc).
The Center for Advanced Wound Healing and Hyperbaric Medicine is staffed so as to offer the highest quality of services available to the patients, medical staff and community. Staffing includes physicians, podiatrists, registered nurse, licensed practical nurse, nurse technician, and hyperbaric technicians. The department’s budgeted positions are determined annually during the budget preparation period, and are modified when increased volume or new program needs are anticipated.
Qualifications for each staff position are included in each job description. The department provides services Monday through Friday from 8 AM to 4 PM.
Extent to Which Care Provided Meets Patients’ Care Needs
The quality of patient care is monitored on a regular basis through the department’s performance plan. Indicators that measure various components of patient focused and organizational functions, as defined by Joint Commission, are used to assess current performance and improve patient care. Patient perception of care surveys and annual staff performance competency evaluations are also utilized to identify opportunities for improvement and educational staff needs.
